Cycle Notes tab


     Each cycle has it's own cycle notes sub-tab. Every time a patient starts a cycle, there is a corresponding cycle note tab. You can navigate between all the cycle notes by clicking on the sub-tabs at the bottom of the screen. They are identified by the cycle type and the sequential number of cycles. You cannot change the data directly on this screen. To change information, click on the "Modify" button on the right side of the screen. Stimulation orders, Notes and Biology orders are entered by clicking on the "Modify" button on the left side of the screen (it is vertical). The cycle type, number and month of cycle (in red) are taken from the corresponding cycle result sub-tab for this cycle number. If the cycle result tab does not exist, the tab will show the cycle type from the cycle check list until a cycle result sub-tab is created. If the Cycle check list does not contains a cycle type, it will show "None Created". The cycle number will always be sequential to match the cycle result sub-tab.

     This "Modify" button will put this screen in edit mode and will let you change the data for the following fields: Procedure Notes, Communication Note and all the data inside the "Consents and Misc." box. If you check the pregnant check box, it will show "Pregnant" in red on this tab and on the cycle results screen. If you check the "Global Insurance" check box, it will show "Global" on the cycle results screen and "Global" in red on the Physician's order dialog window.

     The Communication Note will appear in the "Edit day" window on the cycle results tab as a nurse communication note. The physician will be able to see it by clicking the "View nurse..." button on the physician orders window. Procedure notes are in free text. Most fields are self explanatory.

     When in edit mode, you can't navigate to other parts of the chart. You have to "Save" or "Cancel" before you can navigate. You can click "Save" to store your changes or "Cancel" to return to the previous state and loosing your changes.
 


"Modify" button (left)

     This modify button will bring up a dialog window to enter the stimulation orders, notes and biology orders. This information needs to be signed, that's why it is separated from the other part of the sub-tab. When the dialog window appears, enter your user name and electronic signature to enable the editing fields. You will also need to choose in a combo box from a list of the practice physicians for the ordering physician. The dialog box hold 2 list boxes, one for the list of stimulation types in the system table (to choose from) and a second with the list of patient's ordered stimulations. Most of time, the data you need will be there, but if it is not, you will have to create a new entry in the stimulations table (See Tables). But make sure it is not already there to avoid duplication of data in the tables. You cannot manually enter this information.

     BEFORE ADDING THE FIRST ENTRY, make sure that this is the good cycle number. Once data is entered, you will not be able to remove the tab or change the cycle number.

     To move a stimulation order from the list on the left to the list on the right, you can double click on it, it will copy it immediately. Or, you can highlight the item and click on the "Add->" button. To remove an item from the list on the right, you can double click on it, it will remove it immediately. Or, you can highlight the item and click on the "<-Remove" button. To remove ALL stimulation orders from the list on the right, just click on the "Remove ALL" button.
 


     The stimulation orders and biology  orders boxes on the bottom are free text that will be reformatted after saving to be added in the global field on the sub-tab. You need to click on the "Save" button to keep your changes or "Cancel" to return to the previous state and loose all your changes.

     If you added text in one or the 2 text boxes, it will be added to the global fields on the cycle notes sub-tab. The data entered in the global fields is displayed in chronological order with the most recent on top. The text format is: Date, Username, ordering physician and the text you entered. Once the order is entered, it cannot be erased. Changes or additions may be made and will appear in the global fields.

     This button will bring up a dialog window where you will be able to change medication labels on the cycle results screen (See Cycle Results tab). The medications are rows in the cycle result screen. They default to their respective name, until you change them in this dialog window. You need to click on the "Save" button to keep your changes or "Cancel" to return to the previous state and lose all your changes.

Create a NEW Cycle Notes sub-tab

     The Cycle Notes tab contains one sub-tab for each cycle. Every time a patient starts a new cycle, a new sub-tab must be created to enter information for the new cycle. To create a NEW cycle notes sub-tab:

  1. Click on the "Cycle Notes" tab.
  2. Click on "edit" in the pull-down menu.
  3. Click "Create a NEW Cycle..." in the pull-down menu.

This will open the Create/Mod. dialog window. (Following picture)

     A dialog window will open where you can modify the cycle number, create a new cycle notes sub-tab or delete the selected sub-tab. You select what you want to do by clicking on the radio button for this option. A radio button is a "one ON only" type of button, meaning that only one button can be selected at a time within a group of buttons inside a gray box. When you select one, the others are automatically not selected.


     When you select "Modify the SELECTED Tab Cycle Number", the "Enter Cycle Number to link the notes:" field is enabled and selected for you to change this number. The label text on the "OK" button will become "Save Modifications". If you change the cycle number, it MUST link to an existing cycle result sub-tab. Also, you cannot change the cycle Notes sub-tab cycle number if there is already one with the same cycle number. If you try to duplicate it, you will receive an error message and the change will not be performed. You need to change the cycle number only to fix a mistake after a recent creation. When it is used and data is stored, you should not change the number.
 


     Selecting "Create a NEW Cycle notes tab" radio button causes the text label on the "OK" button to turn to "Create NEW Tab" and the "Update Previous Cycle in the Check List" check box enables. This box, when checked, will move the cycle description in the check list to the "Previous Cycle" box. The cycle history is then automatically be written to the new cycle Checklist (See Cycle Check List tab).

     BEFORE CREATING A NEW CYCLE NOTES TAB, make sure that there is no tab already created for the cycle number you need.

     The data in the "Data from Check List" shows you actual values entered. The "Cycle Type:" shows the type defined in the combo box in the check list tab. If it shows "NO CYCLE CHECKLIST" in red, it means that there is no check list created or that the combo box for the cycle type is empty. Again, you cannot create a new tab with the same cycle number than an existing one.
 


     Selecting "Delete the Selected Tab (Permanently)" radio button causes the text label on the "OK" button to turn to "DELETE Selected Tab". If you choose this option, be aware that all the data will be permanently erased after this operation. The program will prompt twice if you wish to delete this cycle note (as a security measure). You will not be able to delete a tab if it contains Stimulation orders or Biology orders for medical legal reasons. If you try to delete such a tab, you will receive an error message and your operation will be aborted.
