| The patient's name , ID number, date of birth, age,
date of next visit, patient's picture and some flags to classify the record are always visible on the screen. Only the
tabbed data changes when you navigate inside the chart. Also, name in black designate a female; name in blue designate a
male. In red, just under the "Next visit" field, are the hospital affiliation and the practice for this
patient.
The first page of the chart is the demographic information tab.
On this page you will find useful information like address, phone numbers, marital status, partner and employer. If you
need to modify the information on this page, you have to click on the appropriate "Change..." button in
the area you want to edit. This will open a dialog window where you will be able to perform your changes. If you try to
perform changes directly on the chart, you will get an error message. |

To select dates, either type directly in the field or click on the arrow to get a calendar. The partner ID must be entered for the link buttons (see below) to work. The Active flag is to separate patients under active treatments from patients not currently in treatment. The Monitoring room report flag for patients being monitored for blood levels. The mailing list flag is to include or remove the patient from the mailing list (if unchecked). |

"Partner:" and "Switch to partner..." buttons
In most charts, these 2 buttons will bring up the chart of the
patient's partner. If both partners are configured correctly, the user can move very fast between the 2 charts to
compare data. The upper left "Partner:" button can also change to a third chart for donors (See
Donors). The label on this button can change according to the "Label on Button:" field in the medical
information tab. Then, the field "Other Pt. Link:" on the medical information tab is the patient ID number
where this button will point to. If there is no partner setup for a patient, a
new button appears: Create Partner... This button, if pressed, will create a new
chart with nearly the same information then the actual chart, but for a partner,
you just need to enter the new name and a new chart will be created. This saves
a lot of entry time to the employees. |

"Change..." button for address This button is use to modify the patient's address and the phone
numbers. When you click on this button the following dialog window appears: |

Most of the fields are self explanatory. The city, state and country fields are combo boxes. When you click on the little arrow to the right of the field, it will show a list of choices already entered in the system tables. Most of time, the city you need will be there, but if it is not, you will have to create a new entry in the city table. But make sure it is not already there to avoid duplication of data in the tables. You should ask the staff responsible for this administration function. You cannot manually enter these information fields. When entering the phone numbers, you only have to type the digits, parentheses and dashes are positioned automatically. If the patient consents that medical personnel call home or call at work to leave messages, just check the appropriate box (es). The instructions text boxes are for special phone instructions for the staff. "Change..." button for marital status To enter the marital status, click on the radio button for the
option you want. A radio button is a "one ON only" type of button, meaning that only one button can be
selected at a time within a group of buttons inside a gray box. When you select one, the others are automatically not
selected. |

In the "View" drop-down menu, you can click on "View Patient Picture..." to view the large patient's picture (See Scanning Documents). You will see the following dialog window. To close it, just click on the X in the upper right corner. |
