Medical Information

    The medical information tab contains information related to patient's physical / history data. Inside this tab you have 3 sub-tabs folders; "Physical info", "Physicians, Hospitals and History" and "Egg donation program notes". The physical info page looks like this:
 

     If you want to modify the information on this tab, you click on "Change..." button in he middle of the page. If you want to modify the allergies list, click on the "Allergies..." button on the upper right. Both buttons will bring up a dialog window where you will edit the information.

"Change..." button

     The only place to modify the information on the physical information is via this button. If you try to directly edit the data on the screen, you will get an error message.
 


    The Height and Weight fields can be entered in U.S. measures or in metric measures. If you choose to enter in the metric side, the other side will calculate automatically. You will always have the values in the 2 measurement methods. The Body Mass Index (B.M.I.) will be calculated automatically according to the values entered. This information will also be displayed in the physician orders screen.

     The 3 check boxes are for Rubella immune, smoking patient and Latex allergy. The latex allergy is special because it will print on the test tube labels to communicate this important information to the clinical staff prior to draw the blood.

     To choose the sex, click on the radio button for it. A radio button is a "one ON only" type of button, meaning that only one button can be selected at a time within a group of buttons inside a gray box. When you select one, the others are automatically not selected.

     Most of the fields are self explanatory. The blood type and the primary nurse fields are combo boxes. When you click on the little arrow to the right of the field, it will show a list of choices already entered in the system, most of time, the data you need will be there, but if it is not, you will have to create a new entry in the table (See Tables). But make sure it is not already there to avoid duplication of data in the tables. You cannot manually enter this information.

     In the pregnancy status section, the box labeled "Notes:" and the current medications box are large memo text fields where you are not limited in length for your entry. You can scroll the text with the vertical scroll bars on the right side of the field.

     There are 2 check boxes for communication purpose. Checking any of these will force the current chart to appear in the flow of charts to review by physicians or by nurses according to which check box is selected.

Donor, Recipient, Carrier and Biologic Mother

     The "This Patient is Anonymous" check box is to flag a patient to be anonymous. When checked, the name appearing on the top of the chart will be the one in the "Name to show on chart:" field. This is normally an internal name code to hide the real patient's name. If the user has the "View Donor" rights (See Security), the full name will be displayed, If the user don't have the "View Donor" rights, he will see the internal name code only.

     The "Pt. ID to link the chart to:" field is the ID number of a third chart to link with this patient, like a donor or recipient. The "Label to show on the button" field is the text that will appears on the button just under the patient name to identify what kind of link it is. Normally, it could be "Donor", "Recipient" or any other short text that needs to show on the button.

     If none of these fields are entered, the button will show "Partner:" and the link will be with the partner.

"Allergies..." button

     This is used to list the patient's allergies. The dialog window holds 2 list boxes, one for the list of allergies in the system table (to choose from) and a second to list patient's allergies. Most of time, the data you need will be there, but if it is not, you will have to create a new entry in the allergies table (See Tables). But make sure it is not already there to avoid duplication of data in the tables. You cannot manually enter these information.
 


     To move an allergy item from the list on the left to the list on the right, you can double click on it, it will copy it immediately. Or, you can highlight the item and click on the "Add->" button. To remove an item from the list on the right, you can double click on it, it will remove it immediately. Or, you can highlight the item and click on the "<-Remove" button. To remove ALL allergies from the list on the right, just click on the "Remove ALL" button. You need to click on the "Save" button to keep your changes or "Cancel" to return to the previous state and loose all your changes.
