List of Patients by Primary Physician

     The "Choose Type" box permits you to select the type of report filter; by referring physician or by primary physician. Here you select Primary.

     The "Date of FIRST visit" box is for filtering between 2 dates on the date of first visit. This information must be entered in the records for this filter to work. You have a "From:" and a "To:" date field to build your query. IF YOU DO NOT WANT DATE FILTERING, you MUST uncheck the boxes to the left of the date. Not clearing the flag will take the selected date (today's date as default) as the filter for the report.  These are 2 date controls where you can specify the date filtering for the report. You can type the date directly or click on the drop-down arrow to the right of the field to have a calendar to choose from. You choose a date by clicking on the number for the day you want. The selected date will appear in a blue circle. Date is entered by clicking in a calendar where you can find the date you want. Clicking on the arrows will move from month to month. If you click on the name of the month on the calendar, a drop down menu will appear, giving all of the available months. Clicking on the year will produce 2 small up-down arrows, allowing the change of year . The selected date is shown in the blue circle. Clicking on the red circle at the bottom left will bring the calendar to today's date. If no date is entered in a field, by default, the date control has today's date.

     The "Select a Physician filter" box is a multi-selection list box where you can click on a line to select it. You can select many lines as clicking on a line will NOT unselect the others. To unselect a choice, you just click on the line again. This is a list of physicians from the system table displayed by alphabetic order. The report will list patients with the selected physician (s) as a primary physician only. If you do not select a physician, the list will become a list of patient between the 2 selected dates of first visit. It will include ALL the physicians.

     Clicking on the "Process..." button will start the report generation. The process will build the report in memory. When the process finishes, FertiSoft will bring you to the print preview screen where you can visualize the listing as if it were on paper. From this preview, you can then decide to print it on paper or not. Clicking on the "Choose the output Printer" checkbox will activate a dialog window to choose your printer BEFORE it is previewed. This is useful if you want to change the target printer or change some of the printing parameters (See Printing Dialog).

     The layout on this listing is data retrieved from the patient database for the filters you chose and only patients matching the filters will appear on the list. This is a summary listing showing standard demographic information.

You have useful information on this report, from left to right:

• Physician's ID number in system table
• Physician's name
• Physician's phone number
• Physician's office address

Then each patient is listed like:

• Patient's ID
• Name
• Date of first visit at the practice
• Active flag (X means checked)
• Date of birth
• Home phone number
• Can leave message flag (X means yes)
