Satellites
This is the list of satellite locations for use in the combo boxes where the user needs to
enter or to choose a satellite locations. The database has fields for the office address, phone and fax numbers. This is
also where you write the personalized text for the master satellites report. This database can also be used to create a
mailing list out of the satellite location names.
Physicians
This is the list of physicians for use in the combo boxes where the user needs to enter or to
choose a physician. The database has fields for the office address and phone number. This database can also be used to
create a mailing list out of the physician names.
Pharmacies
This is the list of pharmacies for use in the combo boxes where the user needs to enter or to
choose a pharmacy. The database has fields for the business address and phone number. This database can also be used to
create a mailing list out of the pharmacies names.
Insurance companies
This is the list of insurance companies for use in the combo boxes where the user needs to
enter or to choose an insurance company. The database has fields for the office address and phone number. This database
can also be used to create a mailing list out of the insurance company names.
Hospitals
This is the list of hospitals for use in the combo boxes where the user needs to enter or to
choose an hospital. The database has fields for the office address, phone and fax numbers. This database can also be
used to create a mailing list out of the hospital names.
Cycle types
This is the list of cycle types for use in the combo boxes where the user needs to enter or to
choose a cycle type. The database has fields for the short and long description of the cycle for use in reports.
Stimulation types
This is the list of stimulation types for use in the combo boxes where the user needs to enter
or to choose a stimulation type. The database has one field for the description of the stimulation for use in reports.
Laboratory tests
This is the list of laboratory tests for use in the combo boxes where the user needs to enter
or to choose a laboratory test. The database has fields for the description of the laboratory test for use in reports,
the type of test and the number of test tubes for this test.
Allergies
This is the list of allergies types for use in the combo boxes where the user needs to enter or
to choose an allergy. The database has one field for the description of the allergy for use in reports.
Blood codes
This is the list of blood types for use in the combo boxes where the user needs to enter or to
choose a blood type. The database has one field for the description of the blood type for use in reports.
Diagnosis codes
This is the list of diagnosis codes for use in the combo boxes where the user needs to enter or
to choose a diagnosis. The database has one field for the description of the diagnosis for use in reports.
Physician's order codes
This is the list of physician's order codes for use in the combo box where the physician needs
to enter an order. This is useful to limit the typing to a minimum in the order box. The database has one field for the
text to use in the order box..
Towns
This is the list of towns for use in address combo boxes where the user needs to enter a town.
The database has one field for the name of the town, one field for the state and one field for the default zip code for
this town (for small towns).
States
This is the list of states for use in address combo boxes where the user needs to enter a
state. The database has one field for the name of the state.
Countries
This is the list of countries for use in address combo boxes where the user needs to enter a
country. The database has one field for the name of the country.
Relatives
This is the list of relative types for use in the combo boxes where the user needs to enter a
relative type. The database has one field for the description of the relation.
Employees
This is the list of the practice employees for use in the combo boxes where the user
needs to enter or to choose an employee. The database also have fields for the employee's address and phone number.
There is also a link (User name) between the FertiSoft users and employees. There is 2 main types of employees; Nurses
and Doctors. This is also where the electronic signature resides. This database can also be used to create a mailing
list out of the employees names.
Folders
This is the list of folders to use in the scanning documents. The database has one field for
